Each new IDEA eligible student in the district requires a Consent to Bill Medicaid form to be completed upon arrival. The Consent to Bill Medicaid form is required for each student and lasts the duration of their enrollment (not the school year). Parents or guardians have the option to revoke Consent to Bill annually. If a student leaves the district and re-enrolls, a new form needs to be completed. This form enables the district to bill for portions of any evaluation and certain services such as Speech, Occupational, and Physical Therapy, personal care and busing.
